Recognises an object in the SQL missing from the model

What it means

A table, view, index, sequence or trigger in `08-ddl/` with no counterpart in the model. It was added for one urgent reason and is now invisible to every later review, every other dialect and every new environment.

How it is assessed

Flags the deficient case and passes the acceptable one. Deficient: `ix_fact_payment_value_date_key` created by hand on Postgres to rescue a month-end report; it is in no other target's output and in no model, and the next environment is built without it Acceptable: The index in the model's `indexes`, so it appears in every target's output — commented out where the engine has none (row 13) — and in the ERD
